The CDC Worksite Health ScoreCard is an online tool which can assist you, the employer, in identifying gaps in your health promotion programs, and helps to prioritize high-impact strategies for health promotion at their worksite including: organizational supports, tobacco use, nutrition, physical activity, weight management, stress management, depression, high blood pressure, high cholesterol, prediabetes and diabetes, heart attack and stroke, material health and lactation support, vaccine-preventable diseases, and occupational health and safety.

State agencies who are under the purview of State Employee Wellness Program will be recognized from the internal process using the CDC Health Scorecard for the Recognized Healthy Employer and Healthiest Workplace Awards.
